This cake was good, but why doesthe chocolate end up in small chunks that didn't even bake out? Same with the frosting...what am I doing wrong?

A little bummed about the cake. I baked it for my daughter's first birthday. It was rich and chocolatey but not as moist as I had hoped, especially considering there was sour cream in the recipe.
